Nova Scotia Health Authority’s Palliative Care Guide is available online for all health care providers to find tools and resources to help provide a palliative approach to care. It includes information about:

·         Identification (surprise question, SPICT tool, etc.)

·         Assessment (PPS, ESAS, etc.)

·         Care planning (symptom management, advance care planning, personal directives, etc.)

·         Community support programs (EHS Special patient program, Palliative Home drug program etc.)

·         Learning opportunities (including updated LEAP schedule, upcoming webinars etc.)

·         Patient and Family Resources  (which can be printed at the point of care)
